Lisa Renehan has been a Care Co-ordinator within Central Halifax PCN at Pennine GP Alliance (PGPA) for four years. Her role focuses on supporting care homes and ensuring residents receive coordinated, safe, and effective care through strong communication and organisation.
Lisa particularly enjoys the relationship building aspect of her role. “My role is mainly working with the Central Care Homes, I enjoy engaging with the homes, building up meaningful relationships. We are often the link between healthcare professionals and do lots of work to make the process run smoothly.” Her day-to-day work is varied, ranging from managing prescriptions and registering new patients to supporting homes during outbreaks and coordinating vaccination activity. She has also been actively involved in cancer screening, speaking to patients, booking appointments, and attending screening events.
Strong communication, empathy, and organisation are essential in Lisa’s role. She works closely with care homes, arranging medication reviews, Personal Plan Reviews, and Dementia reviews, while also coordinating and chairing Care Home and Ageing Well MDT meetings. “Teamwork is also a good skill as I am always working with different teams,” she explains, highlighting the importance of collaboration in delivering joined-up care.

Lisa joined PGPA following redundancy from Lloyds Banking Group during the Covid-19 pandemic, where she had worked for 18 years. She initially supported GP practices with organising vaccination clinics and quickly progressed into a lead role. “I then went on to help the practice arrange all the covid clinics and was soon made the lead for arranging all the clinics, arranging the vaccinators, volunteers and making sure all the clinics ran smoothly.” This experience led to her current position as a Care Co-ordinator.
One of Lisa’s proudest achievements has been developing structure and processes for care home support. “When I first started, I was thrown in to working with the Care Homes there was no real structure, and I quickly had to get processes in place.” She introduced improvements such as setting up ward rounds and monthly MDT meetings, creating a shared inbox, and supporting care homes to access online systems. “I feel now it is working very well and I am proud of all the work I have done to achieve this.”
Outside of work, Lisa enjoys spending time with family and friends and walking her dog, Teddy. She credits her team for their ongoing support.
