Supporting you back to work
The Healthy Working Life, Health and Growth Accelerator programme in Calderdale is designed to help people return to work by supporting their health and wellbeing.
If you are currently employed but off sick with a Fit Note due to:
- Mental health concerns (such as stress, anxiety or depression), or
- Musculoskeletal (MSK) conditions (such as joint, back or muscle pain)
you may be eligible for support through this programme.
How it works
If you are referred into the programme, you will be contacted by a friendly Healthy Working Life Advisor from Pennine GP Alliance.
They will:
- Listen to your situation
- Provide advice and guidance
- Connect you to the right support services
This may include a range of NHS services and local voluntary and community support to help you manage your health and return to work safely.


Our role at Pennine GP Alliance
Pennine GP Alliance is supporting the delivery of this programme in Calderdale.
We are:
- Helping identify and refer eligible patients
- Providing dedicated Healthy Working Life Advisors
- Working with local GP practices and personalised care teams
While we are not the direct provider of the service, we play a key role in helping people access the right support at the right time.
How to access support
If you are off work due to a Fit Note for a mental health or MSK condition, speak to a healthcare professional about the Healthy Working Life programme.
They can help determine if this support is right for you and make a referral.
A collaborative programme
Healthy Working Life is a joint programme delivered by the West Yorkshire Combined Authority and NHS West Yorkshire Integrated Care Board.
